I've seen it done well, but it does take a lot of work, especially in the ones where Picard likes Q back. Trust is a big issue. But I think some of the draw for the writers is that it spawns all kinds of interesting storylines for Picard to show up anywhere in time and/or space in a story; Q makes a great deus-ex-machina that way.
