Yeah, that's the one big casting WTF for the new Star Trek. Nothing wrong with Urban, great actor, but physically bears little resemblance to the original McCoy. He's much broader, for one thing, and the head shape doesn't match.
(Maybe since he's the Doctor, they could pass it off as regeneration?)
The guy from Heroes that they cast as the young Spock, on the other hand, PERFECT choice!
ignavus suggested, below, that it was probably more of a personality match than a physical match. And if I think about it, I agree; he probably gave a great reading. But when I think "Karl Urban" I picture The Reaper, Lord Vaako, Éomer Éadig. Maybe the promos will convince me.

I'm glad none of them are huge names. It's going to be hard enough living up to the original cast, nevermind bringing their own ego-overhead into it.
I have a sneaking suspicion my favorite character from the new cast is no longer going to be Kirk, though. =)
